ONLINE EVALUATIONS ARE NOW AVAILABLE VIA ZOOM. IF YOU CHOOSE AN ONLINE APPOINTMENT OPTION, YOU WILL BE EMAILED THE CONSENT FORM AND PERSONAL MEETING ID 24 HOURS PRIOR TO YOUR APPOINTMENT.

PLEASE NOTE, NO ONLINE OR IN OFFICE CHILD EVALUATIONS ARE OFFERRED DURING COVID FROM 2020 to 2022. Please contact Dr. Anna Tamis for child and adolescent evaluations at www.drannatamis.com.

Dr. Balada only accepts Aetna, and Cornell student health insurance. NO OTHER INSURANCE IS ACCEPTED. If you carry other insurance, you will need to private pay for services. A credit card is required to schedule an appointment, but your card will not be charged until after you meet with Dr. Balada. You will have a chance to approve the copay and/or total amount due before the evaluation begins.

Please enter your Insurance ID number and your birthdate in the "appointment notes" section when you book.

If you are a Cornell or Ithaca College Student, you need only schedule a 2 hour Student Assessment appointment. No other appointments are necessary for students. If you are an adult seeking Psychological Testing, please schedule a two-hour Psychological Assessment session and Dr. Balada may add another 2 hours on a different day after she meets you.
